Required Paperwork

The following additional paperwork, based on grade level, may be required to be submitted before the first day of school as noted below.

Physical Examination

Required for:

  • all preschool students
  • kindergarten students NOT previously enrolled in the district’s free four-year-old preschool program
  • students attending school in the United States for the first time (grades 1-12)

NOTE: students will NOT be allowed to attend school without this document on file with the district

Immunization Certificate

Required for:

  • preschool, kindergarten, grades 7 and 12
  • students attending school in the United States for the first time (grades 1-12)

NOTE: students will NOT be allowed to attend school without this document on file with the district

Dental Screening Certificate

Required for:

  • kindergarten and grade 9

NOTE: dental screenings are provided at school for students in kindergarten and grade 9 if they have not had a dental screening and/or returned the dental screening certificate

Vision Screening Certificate

Required for:

  • kindergarten and grade 3

NOTE: vision screenings are provided at school in the fall for all students in kindergarten and grade 3, which fulfills this requirement

Drop off a printed copy to the district enrollment office at 2300 Chaney Road

Request your student’s healthcare provider fax a copy to the district enrollment office at 563/552-3006
